What Is Informed Consent?

Knowledge is power and Informed Consent is the cornerstone of Patient Advocacy. Nurses sign and bear witness that patients FULLY understand all of the risks and benefits BEFORE they sign the Informed Consent Form.  Informed consent is a collaborative process intended on bringing patients and healthcare providers together to make decisions when more than one reasonable alternative exists, accounting for the patient’s unique individuality, preferences and priorities while considering all the best scientific evidence available.

The following are the required elements for documentation of the informed consent discussion:
(1) the nature of the procedure,
(2) the risks and benefits and the procedure,
(3) reasonable alternatives,
(4) risks and benefits of alternatives,
(5) assessment of the patient’s understanding of elements 1 through 4. Discussion of all risks is paramount to informed consent in this context.
